Change of Address or Doing Business
as ("DBA") Name, Vehicle, or Vehicle License
Number
(a) Registrants shall
report any proposed change of address at least twenty
(20) days before such change to the Division of Labor
Standards Enforcement, Licensing Unit. Failure to notify
the Division of an address change voids the registration.
(b) Registrants shall
report any proposed change of a doing business as ("dba")
name at least twenty (20) days before such change to
the Division of Labor Standards Enforcement, Licensing
Unit. Failure to notify the Division of a doing business
as ("dba") name change voids the registration.
(c) Registrants shall
report any change of vehicle or vehicle license number
within twenty (20) days of such change to the Division
of Labor Standards Enforcement, Licensing Unit. Failure
to notify the Division of a change of vehicle or vehicle
license number voids the registration.
