Contents of Registration.
Registration
shall show:
(a) Name and
address.
(b) Age.
(c) Height.
(d) Weight.
(e) Color hair.
(f) Color eyes.
(g) Relationship
to producer--i.e., father, mother, son, daughter,
or regular employee.
(h) If an employee:
(1) Capacity in which employed; and
(2) Date employment commenced.
