§12022. Filing
of Application for Hearing to Determine Controversy.
Proceedings shall be commenced by filing
at the office of the Labor Commissioner, Licensing
Division, P.O. Box 420603, San Francisco, CA 94142-0603,
a petition to determine controversy between the
artist and talent agency, which shall set forth
the claim or demand of the petitioner and shall
be signed by the petitioner or a person duly authorized
to act for him and shall set forth:
(a) A statement as to the nature of the controversy,
including submission of such pertinent information
as is within the knowledge of the petitioner.
(b) The claim or demand of the petitioner.
(c) A copy of any contract pertaining to the controversy.
