s 1300.51.2. Consent to Service of Process.
The consent to service of process required pursuant to these rules shall
be in the following form:
TO THE DIRECTOR OF THE DEPARTMENT OF MANAGED HEALTH CARE OF THE STATE OF
CALIFORNIA CONSENT TO SERVICE OF PROCESS
KNOW ALL MEN BY THESE PRESENTS:
That the undersigned, __________, (a corporation organized under the laws

__________) hereby irrevocably appoints the Director of the Department of
Managed Health Care of the State of California, or his successor in
office, to be his (its) attorney to receive service of any lawful process
in any noncriminal suit, action or proceeding against him (it), or his
(its) successor, executor, or administrator which arises under the
Knox-Keene Health Care Service Plan Act of 1975 or any rule or order
thereunder after this consent has been filed, with the same force and
validity as if served personally on the undersigned.
For the purpose of compliance with the Corporations Code of the State of
California, notice of the service and a copy of the process should be sent
by registered or certified mail to the undersigned at the following
